NEW PATIENTS: A credit card is required to book but will not be charged unless you no show or cancel within 24 hours and then the late cancel fee of $175 will be charged.

WAITLIST: To put yourself on the waitlist, click link at bottom left of the schedule. If you want to be on the list for both locations, you need to create two waitlist entries (one for each location)

EVALUATIONS: PLEASE SCHEDULE A FOLLOW-UP 2-4 WEEKS AFTER EVALUATION SO YOU HAVE IT AND WE CAN DISCUSS AT EVALUATION
